COMMUNITY INTEGRATION SERVICES
Ministry of Social Development and Poverty Reduction
Community Integration Specialists help people at risk of or experiencing homelessness:

  • Access government programs
  • Get connected to services and supports, such as cultural, health and community supports
  • Work with governments, community partners and agencies
  • Provide support during critical transition points, such as leaving a medical or corrections
    facility
    Community Integration Specialists help people access financial support and community
    resources, including:
  • Assistance applying for income
    and disability assistance
  • Funding for urgent needs,
    such as food or clothing
  • Case management
  • Housing support referrals
  • Health support referrals

Community Integration Specialists work with community partners to help people access culturally appropriate services. The Specialists work together to connect people with what they need when and where they need it.

Community Integration Specialists work
where individuals in need are, including:

  • Encampments
  • Family and youth organizations
  • Friendship centres
  • Corrections and courts
  • Mental health and addiction service
    locations
  • Drop-in centres
  • Community shelters or outreach programs
  • Settlement agencies

Referrals can be made by a parent or guardian, family doctor, community workers and agencies, school staff, and correctional facilities.
